CAOT memberships run for 12 consecutive months from the date of registration/renewal. Membership fees are non-refundable.

To purchase Professional Liability Insurance with your Individual Associate registration/renewal, you must complete the Individual Associate Insurance Validation form. Each CAOT occupational therapist member with whom you are working must validate your insurance. *** This must be completed yearly by each of the supervising CAOT members for Individual Associate insurance to be valid. ***
Additional supervising CAOT members may be named by completing and submitting an additional copy of this form at any time during the membership year. This form must be received at CAOT national office via mail, fax, or e-mail, or downloaded to your profile directly through the online renewal/registration process.
