Position Details 

Available Territories: 
  • Fraser Valley - Langley, Abbotsford, Chilliwack

This is a part-time to full-time community-based contract position. You will be working with a diverse group of funders to assist clients with a range of limitations. Health populations and case mix can be customized based on your interest and experience. Duties include Functional Assessments, Job Site Visits and development of Graduated Activity and/or Graduated Return to Work plans as well as liaising with the client’s healthcare team and payer representative. You will use your exceptional communication, clinical and organizational skills to deliver outcomes that change peoples lives by helping them function more fully in the settings that matter to them, fulfill important life roles and return to the activities that give them purpose. Many of our clients require a period of Active Rehabilitation to increase independence in home and community settings before RTW can be considered. You will develop knowledge of funder contracts and community resources to facilitate your client’s access to appropriate health services and will implement a variety of therapeutic strategies to facilitate your client’s success. For clients recently discharged from hospital or those with ADL difficulties, Home Assessments to determine equipment, support and attendant care needs also form part of the duties. Health populations include Orthopedics, Mental Health, Neuro (e.g. Concussion/Brain Injury) and Persistent Pain. 


Qualifications
  • Excellent communication skills both verbal and written
  • Strong organizational skills and attention to detail
  • Reliable vehicle to use for clinical appointments
  • Experience working with agency payers is a strong asset
  • Commitment to continuous learning through professional development courses
  • Current member of COTBC or eligible for registration
