Adolescents are expected to know how to self-advocate, self-manage and be autonomous when they transition into adulthood, however these skills are rarely explicitly taught and developed through the childhood and adolescent life course. This is even more prominent for adolescents with disabilities. This webinar will highlight the occupational therapist’s crucial role in the development and the promotion of skills to facilitate transition planning. Relevant theoretical background and helpful evidence-based tools that can support occupational therapists with transition planning within an educational context will also be shared.

After taking part in this webinar, participants will be able to:

  1. Understand significant theories and models that describe transitions.
  2. Be aware of at least 3 evidence-based tools that can be implemented in their occupational therapy practice.
  3. Critically appraise their role in transition planning and begin to think about how they might incorporate theories, models and evidenced-based tools in their current practice.
